The Senior Care Coordinator oversees the coordination of client services within the healthcare 
facility, managing a team of Care Coordinators, Enrollment Specialists, and Housing Specialists. 
This role involves ensuring compliance with HUD regulations, optimizing resource utilization, and 
leading quality improvement initiatives. The ideal candidate will have a strong background in social 
services, case management, or a related field, along with leadership experience and a passion for 
helping vulnerable populations.


ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Conduct assessments and program intakes for individuals seeking rapid re-housing services. 
  • Ensure data is accurate and complete and that all required accompanying documentation is collected. 
  • Update case notes in electronic case management system in a timely manner. 
  • Participate in the intake and orientation process of new program participants. 
  • Provide direct case management services in the office and on field/home visits.
  • Provide hands-on assistance to enable clients to find suitable housing, including accompanying clients to apartment viewings and conducting neighborhood research throughout NYC and surrounding areas. 
  • Advocate on behalf of the veteran with landlords and brokers to overcome any issues or challenges to securing suitable housing. 
  • Work collaboratively with community housing specialists to identify suitable housing. 
  • Conduct housing inspections for identified units prior to financial assistance being requested. 
  • Mediate household and/or landlord-tenant disputes that threaten housing stability. Serve as point-of-contact for landlords and maintain positive landlord relationships;
  • Conduct assessments and program intakes for individuals seeking rapid re-housing services. 
  • Develop and update housing stability plans to assist participants to obtain or maintain permanent housing. Identify household’s strengths, needs, abilities, and preferences. 
  • Utilize a variety of interventions to assist participants with overcoming barriers to housing stability, including increasing income and benefits.
  • Manage a caseload of up to 25 or more clients, ensuring high-quality support and timely services.
  • Provide oversight and guidance to Care Coordinators, Enrollment Specialists, and Housing Specialists.
  • Work collaboratively with community housing specialists to identify suitable housing.
  • Perform other related duties and responsibilities as assigned; job tasks and responsibilities may be modified or expanded to meet organizational needs.

JOB QUALIFICATIONS

Education: 
  • Bachelor’s degree in Social Work, Human Services, Public Administration, or a related field (preferred).

Experience:
  • Minimum of 5 years of experience in social services, case management, or a similar role.
  • 3-5 years management and progressive leadership experience in healthcare or related field. 
  • Knowledge of HUD regulations and experience with HUD-funded programs (preferred).

Skill Required
  • Strong organizational skills need to manage diverse responsibilities and provide services at various sites
  • Ability to establish and maintain effective working relationships with colleagues, clients and contracted providers of health care services
  • Ability to plan, organizes, implement, and evaluate needs assessments; action plans; and professional development activities
  • Apply problem-solving skills to resolve issues or concerns regarding health education.
  • Compassion/understanding/desire to work with homeless individuals.
  • Excellent oral and written communication skills needed to provide counseling to clients, document case work and provide training
  • Ability to work independently and within a team environment
  • Ability to adhere to confidentiality guidelines, deadlines and reporting requirements
  • Extremely proficient in Microsoft Office Suite; employee is willing and able to:
  • Create, edit, and interpret charts, tables and spreadsheets (via Microsoft Excel).
  • Effectively manage, filter and sort data (via Microsoft Excel).
  • Create well-organized documents and outlines (via Microsoft Word).
  • Manage mailbox and calendar (via Microsoft Outlook)

Pay: $80,000

  • Bilingual English/Spanish/French strongly preferred.
