Care For the Homeless

JOB SUMMARY: Managing and directing a comprehensive program of support, safety and success for women that include housing, counseling, benefit entitlements, program placement, health and educational programs and comprehensive case management.         
                              
This employee will work as part of the Social Services team with the following essential duties and responsibilities:
 

• Assessing housing history and independent living skills of clients
• Help clients in identifying any negative credit history that may impact their apartment search or issues with brokers and landlords.
• Identifying available housing resources and maintaining an up-to-date directory of these resources
• Individual meeting will focus on tenants’ rights, home maintenance/upkeep, and landlord/tenant relationships
• Maintaining required case records in written and computer generated forms, and reporting on same in regular supervisory meetings
• Maintaining required housing records in Cares.
• Responsible for completion and submission of HRA-2010E application
• Works with all DHS and Housing Entitlements
• Coordinating housing contracts, referrals and follow-up to service providers including but not limited to DHS
• Documenting all counseling, referral and discharge planning activities in client folders
• Participating in case conferences, staff meetings and other meetings/trainings as assigned
• Adhering to DHS timeframes for permanent housing placing
• Attend required DHS meetings
• Having flexible work hours to meet the needs of the clients (ex. Working at least one late night or more if needed).
• Drive agency van as necessary.
• Perform other duties as it relates to the Housing Specialist position                          

• Bachelor degree preferred

• Two (2) plus years’ experience working with mentally ill, medically frail or homeless population
• Experience working with housing entitlements
• Exceptional knowledge of regional housing, supportive housing, Section 8 and other rental assistance programs
• Valid drivers license (Preferred)

• Excellent communication skills, both verbal and written
• Strong computer skills

• Bilingual in English and Spanish strongly preferred.

Outpatient ambulatory care dental hours may include one or more evenings per week. Some additional time may be necessary as the needs of the practice dictates to complete charting and documentation.
